The annual inspection identified multiple deficiencies, including immediate health risks related to water temperature and unsecured bed rails. Non-compliance was also noted regarding general facility maintenance and the completeness of resident care plans. Immediate corrective action is required for the Type A citations.
Faucets used for personal care must deliver hot water, and temperature controls must maintain a temperature between 105°F and 120°F. Water was measured at 84-94 degrees F in the kitchen and resident bathrooms, posing an immediate risk.
Postural supports (bed rails) require a written physician's order. Four out of six residents had bed rails at the head of the bed without the required physician's order, posing an immediate risk.
The facility must be clean, safe, sanitary, and in good repair at all times. LPAs observed the facility was not in good repair, noting issues with bathrooms, floors, and furniture, which poses a potential risk.
The facility must maintain complete appraisal needs and services plans for all residents. Six out of six resident plans were incomplete because they lacked signatures from the facility representative, resident, and/or RP.
