Merced Girard Residential Care Facility

The facility underwent an Annual 1-year required visit on July 10, 2025. The Licensing Program Analyst conducted a thorough inspection of the physical plant, including the kitchen, common areas, and backyard. No deficiencies were cited during this inspection, and the report was reviewed and discussed with the administrator.

This report details an unannounced complaint investigation conducted on 06/05/2025 regarding allegations of neglect leading to resident death and failure to seek timely medical attention. The investigation found that the allegations were unsubstantiated due to a lack of preponderance of evidence.

This report details a complaint investigation conducted on 10/21/2024 regarding allegations of delayed medical attention and staff yelling. After reviewing interviews and documents, the Licensing Program Analyst found that the allegations were unsubstantiated due to conflicting information and lack of evidence.

The annual inspection identified multiple deficiencies, including immediate health risks related to water temperature and unsecured bed rails. Non-compliance was also noted regarding general facility maintenance and the completeness of resident care plans. Immediate corrective action is required for the Type A citations.

The inspection was conducted to review Infection Control procedures and practices. The facility demonstrated good practices regarding COVID-19 precautions, staff training, and general cleanliness. No deficiencies were cited during this inspection.
