Numerous plastic garbage bags (containing resident belongings, ruined items, and empty cans) were left on the front porch, obstructing a designated egress point.
Inoperable washer and dryer were left in the driveway/garage area.
The home is dirty with bags of garbage on the porch, peeling paint, unfinished walls and ceilings, dirty and stained carpeting and damaged flooring. Specifically: unfinished drywall/unpainted walls in resident bedroom; missing/broken floor tiles and mold on ceiling in bathroom; broken tiles in dining room; excessively dirty/stained/worn carpet in NW upper bedroom; torn/worn carpet in upper hallway; unfinished/unpainted ceiling in SW upper bedroom.

One half gallon of milk in a resident refrigerator was opened, dated January 19, 2025, and smelled sour at the time of the inspection.
The written assessment for one resident had not been completed on an annual basis.
