An on-site compliance inspection was conducted on November 18, 2025 and the following deficiencies were cited:

Based on observation, record review, and interview, the manager failed to ensure a medical record was maintained at the facility for one of two sampled residents, which posed a health and safety risk for lack of information provided to caregivers. Findings include: 1. During the environmental inspection of the facility, the Compliance Officer observed eight residents at the facility. 2. A review of R1's medical records revealed a medication administration record sheet (MAR) for November 2025 along with medication orders. However, no additional documentation was provided for R1 specific to orientation of the facility. Based on R1's admit date, the documentation was required. 3. In an interview, E2 and E3 acknowledged there were no complete medical record for R1 at the facility at the time of inspection.

Based on documentation review and interview, the manager failed to ensure a disaster drill for employees was conducted on each shift at least once every three months and documented. The deficient practice posed a risk if employees were unable to implement a disaster plan. Findings include: 1. A review of the facility's work schedule revealed the facility had two shifts: 6:00 am - 6:00 pm and 6:00 pm - 6:00 am. 2. A review of facility documentation revealed a disaster drill conducted on each shift in January 2025, April 2025, and July 2025. However, no documentation of disaster drills after July 2025 was provided for review. 3. In an exit interview, the findings were reviewed with E2, and no additional information was provided.
