Key Review Excerpts
“My 92 year old mother has been here a little over 3 months and loves it . She receives Physical Therapy and 24/7 care that is the absolute best in the Tri-State area. Activities, a caring staff and support system they have gives me peace of mind that my mother is in good hands.”
“I can only give 2 stars and that is for Lori the nurse and the cna’s. I wasn’t very impressed with the staffing …. I felt the facilities was very under staffed on the weekends and I expressed my concerns to the director of nursing and was told that they were staffed to the max by federal guidelines.”
“The room was very unkempt, I had to express concerns to mgmt over the unclean state of it for someone to come clean it and some furniture items were falling apart. It was taken care of fairly quickly afterwards but the area of the facility was just very run down in general”

Inspection History
Medicare Inspection History
3-year lookback · Medicare 2026
Madison Park Healthcare shows a pattern of recurring deficiencies across all three surveys, with 41 total violations but no complaint-triggered inspections. The facility has persistent issues with medication management, fire safety systems, and resident care planning that appear across multiple surveys from 2021-2024. While all deficiencies show correction dates from the provider, the recurring nature of problems in critical areas like pharmaceutical services and safety systems suggests ongoing operational challenges families should carefully consider.
