What this means for your family

While the physical therapy department receives high marks for helping patients regain mobility, the facility has recurring, serious reports of neglect, medication errors, and poor communication. We strongly advise families to visit frequently and unannounced to monitor care levels, specifically checking for room temperature and staff responsiveness to call bells.

Key Review Excerpts

The therapy department took the time to explain how and why my mobility was so limited and what could be done to improve my condition.

Rehab patient · 2024★★★★★

My mother was sent to the Madison Health and Rehab Center... They put her in a room which had no heating and the washroom was shared with 2 rooms. They did not give her the Pregabalin medication stating that they don't have it.

Long-term resident's family · 2026☆☆☆☆

While the staff are friendly I'm not sure anyone communicates. Between all the staff working with my mom no one recognized a problem and it almost killed her.

Families have filed numerous complaints against this facility, resulting in 59 complaint-triggered deficiencies across safety supervision, medication management, and resident care standards. The facility repeatedly struggles with preventing accidents and providing adequate supervision, with these issues appearing in nearly every survey from 2023 to 2025. While all violations are marked as corrected, the persistent pattern of the same problems suggests systemic care quality concerns that families should carefully investigate before making placement decisions.
