What this means for your family

While the facility has recently improved its activities program, the recurring reports of unsanitary conditions and poor communication are significant red flags. If you are considering this facility, I strongly recommend conducting an unannounced visit to observe the cleanliness of the rooms and the responsiveness of the staff firsthand.

Key Review Excerpts

This place is downright filthy. I watched a CNA use a feces covered brush and run it through my grandfathers hair. They leave soiled diapers and linen everywhere and refuse to have any kind of ownership for poor management of patients.

Memory care family member · 2023☆☆☆☆

I have been here for 18 months. I was never interested in activities because I felt like coloring a teddy bear out of a coloring book wasn't fun. We now have an activities director named Toni that I wish we had had from the begining. She is very personable and makes every one feel included.

Long-term resident · 2024★★★★★

My Dad has been there since June. I don't know what they do with his clothes, because he has lost SOOOO many clothes there even though I put up a sign that Family will do laundry.

Families have filed multiple complaints triggering inspections, with recent issues in September 2025 involving abuse reporting and investigation procedures. The facility shows recurring problems with care planning, daily living assistance, and infection control that persist across surveys from 2022 to 2025. While all deficiencies show correction dates, the pattern of repeat violations in core care areas suggests ongoing quality concerns that families should carefully consider.
