Key Review Excerpts

Patient neglect was amongst the 12 pages of citations they received. After 3 days I took my husband out of there.

Memory care family member · 2026☆☆☆☆

My mom was put here as a stroke patient. She arrived at the facility at 11:00a.m. they did not change her until 3:00 pm. I was there one day. My mom buzzed the bell as well as some other patients. I was there for about an hour and one never came.

Long-term resident's family · 2024☆☆☆☆

I’ve seen facilities in crisis, and those that run quite well, so I don’t hand out a five-star review lightly, but four stars would fall short of the care we received.

This facility has a concerning pattern of frequent complaint-triggered deficiencies, with families filing 22 reports leading to violations across resident rights, quality of care, and medication management. The facility repeatedly struggles with providing appropriate treatment according to care plans, protecting residents from abuse and neglect, and ensuring proper pharmaceutical services, with issues persisting across multiple surveys from 2022 through 2025.
