Long-term resident's family · 2024★★★★★

I had to take my mother there for rehab & to make sure her medicines were given to her as instructed by the doctor. I was told the medicine would be delivered by that evening, when I picked her up the next morning, she hadn't received any of her medicine.

This facility has concerning patterns in care planning and medication management, with families filing complaints about notification failures and medical record issues in 2025. The most recurring problems involve resident assessment and care planning deficiencies, medication oversight, and infection control failures that have persisted across multiple surveys since 2018. While the facility reports correcting these issues, the repeated nature of core care problems and recent family complaints warrant careful consideration during any visit.
