This was a Plan of Correction (POC) case management visit conducted by the LPA to clear a deficiency cited on March 9, 2026. The LPA confirmed that the facility received the POC for the Type A deficiency and provided a 'Deficiencies Citations Cleared' letter. No new deficiencies were cited during this visit.

The facility underwent an unannounced annual inspection. The physical tour and review of operational areas, including kitchens and resident rooms, showed general compliance. However, one significant deficiency was cited regarding staff background clearance, which poses an immediate risk to residents.

The unannounced Case Management visit was conducted to follow up on the facility's capacity increase request. The inspector noted that all inspected areas, including 6 bedrooms in Building 1 and all 12 bedrooms in Building 2, were equipped with necessary items and that all auditory alarms functioned properly. No deficiencies were cited during this visit.

The facility underwent an unannounced annual inspection. The inspector noted that the facility was generally compliant, observing adequate food supplies, locked storage areas for medications and cleaning products, and functional safety equipment. No deficiencies were cited during this visit.

The unannounced annual inspection visit was conducted by LPA Manuel Monter and met with Administrator Josephine Arcilla. The facility was toured, and various areas, including food storage, medication storage, and fire safety equipment, were observed. No deficiencies were cited during this visit.
