What this means for your family
LindenGrove Mukwonago offers an excellent environment for social engagement and rehabilitation, but families should be aware of reported inconsistencies in nursing responsiveness and administrative communication. When touring, specifically ask about the current nurse-to-patient ratio on weekends and request a clear point of contact for social work concerns to ensure your loved one's needs are addressed promptly.

Key Review Excerpts
“The facility is clean and well-kept. However, the most important aspect of rating a nursing home is how well the residents are treated and cared for. There are a few very caring CNA’s and nurses, but there is a much greater number of employees there who strip the dignity from the elderly on an on-going daily basis.”
“My 90 year old aunt has been here for rehab several times and is now moving into assisted living there. Very nice and clean facility and staff has always been helpful and willing to answer my many questions.”
“My mom is a new resident at Linden Ridge Assisted Living but has been in and out of Linden Grove rehab many times, as well as the skilled nursing unit more recently. I had no complaints with her care in either area.”

Medicare Inspection History
3-year lookback · Medicare 2026
Multiple families have filed complaints triggering investigations, revealing concerning patterns in abuse/neglect reporting, care planning, and safety oversight that have persisted across recent surveys. The facility shows recurring deficiencies in resident care planning, safety/fire protection systems, and abuse prevention protocols, with some issues like inadequate reporting of suspected abuse appearing multiple times despite corrections. While all violations show correction dates, the repeated nature of core care and safety problems, combined with ongoing family complaints, suggests systemic challenges in maintaining consistent standards.
