The facility failed to ensure the Special Care Unit (SCU) was free of hazards and obstructions. Specifically, hazardous items including a portable fire extinguisher, shampoo, deodorant, body wash, and numbing mouth sore gel were left accessible to residents. Staff failed to monitor or secure these items, which posed a risk to residents with wandering behaviors.

The facility failed to ensure that all staff were tested for Tuberculosis (TB) upon hire. A review of personnel records revealed that one sampled staff member did not have documentation of a TB test following their hire date of September 15, 2017.

The facility failed to ensure the designated Activity Director completed the required basic training for Assisted Living within nine months of employment. Personnel records and interviews confirmed the Director, hired on 04/18/19, had not completed the necessary course by the time of the survey.
