The facility failed to administer medications as ordered for three sampled residents, specifically involving medications for infection, high blood pressure, diabetes, insomnia, and seasonal allergies. For Resident #2, medication orders for Omnicef to treat a rash and URI were not followed correctly, as evidenced by discrepancies between physician orders, eMAR documentation, and the physical count of remaining tablets.

The facility failed to maintain walls, ceilings, and floors in a clean and good state of repair. Specifically, a piece of baseboard was missing from the back hall wall, and dirt and debris were observed on the floors of the male and female common bathrooms.

Specifically, two bedridden residents were found without accessible signaling devices, with one resident's wall-mounted call light located 6-8 feet away from his bed.
