For Resident #2, medication orders for Omnicef to treat a rash and URI were not followed correctly, as evidenced by discrepancies between physician orders, eMAR documentation, and the physical count of remaining tablets.

The facility failed to keep walls, ceilings, and floors clean and in good repair in several common areas. Specific issues included a missing piece of baseboard with exposed plaster in the back hall, a cracked shower tile, and an area of missing flooring exposing cement subflooring in the female bathroom. Additionally, dirt and debris were observed on the floors of the male/female common bathrooms on the back hall.
