The facility failed to ensure quarterly Licensed Health Professional Support (LHPS) evaluations were completed for a resident requiring fingerstick blood sugar (FSBS) checks. Specifically, no evaluation had been performed for Resident #2 since August 23, 2019, despite a physician's order for monthly checks.
