A licensure revisit was completed on 8/22/23 for all previous deficiencies cited on 11/3/22. A deficiency was cited. Based on interview and observation, the residence failed to ensure each care plan detailed specific personal service needs and preferences along with the staff tasks necessary to meet those needs, affecting one of three sample residents (#1). This deficiency was cited previously during a state licensure survey 11/3/22. Although the facility corrected the deficiency, based on the findings below, the facility has not maintained compliance with this regulatory requirement. Findings include:1. Residence policyThe residence' s undated Service Plan policy, read in part, the administrator, or designated representative, develops a service plan for each resident prior to admission. The service plan should address behavioral challenges and any need identified by the family or resident.2. Resident #1 was admitted to the residence on 3/30/21 with diagnoses including Alzheimer' s disease.A care plan, dated 6/12/23, read in part, the resident required toileting assistance and frequently requested to use the bathroom throughout the day. However, the care plan did not mention the resident required a toileting plan or the resident' s behavioral challenges in regards to using the call pendant system extensively.A progress note, dated 7/7/23, read in part, the resident pushed the call pendant a lot. A progress note, dated 7/11/23, read in part, the resident pushed her call pendant constantly and staff explained she did not need to do that. A progress note, dated 7/12/23, read in part, the resident pushed her call pendant a bunch of times. A progress note, dated 7/18/23, read in part, the resident pushed her call pendant 50% of the night. A progress note, dated 7/30/23, read in part, the resident pushed her call pendant quite a bit in the afternoon. A progress note, dated 8/3/23, read in part, the resident was on her call pendant as usual.
