Key Review Excerpts

The physical therapist, Ty, is amazing. He was very helpful, patient, knowledgeable, and kind. Helped my dad to get stronger so he could go home.

Rehab patient's family · 2025★★★★★

The night shift does not care at all if you are in pain they say push call light then someone comes in turns light off says i will tell nurse and then nobody ever comes then when the nurse shows up hours later they you should have pushed it again.

Rehab patient · 2024☆☆☆☆

My 88 year old grandma was there to recover from a fall. She left unresponsive to the ER to be intubated. She got a UTI for the first time because she was neglected by the staff.

4 complaint-triggered

Families filed complaints that led to recent deficiencies in treatment quality and medication management at this facility. The most recurring issues involve medication safety, basic care delivery, and fire safety systems appearing across multiple surveys from 2021-2024. While the facility has correction plans for the complaint-triggered deficiencies, the pattern of repeated problems in core care areas suggests ongoing operational challenges that families should carefully evaluate during visits.
