A relicensure survey with complaint #CO40404, #CO40658, #CO40795 and #CO40841 was completed on 1/21/26. A deficiency was cited. Based on the records review and interview, the residence failed to have at least one staff member on-site at all times who had current first aid for two sample staff (#1, #2), affecting 18 current residents.Findings include:Personnel file review revealed that Staff #1 and #2 failed to have first aid listed on their cardiopulmonary resuscitation certificate (CPR). On 1/21/26 at 1:58 p.m., the acting administrator stated she was not aware of the regulation change requiring CPR/First aid to be provided by a nationally recognised organisation. The acting administrator stated that she thought the last CPR class had included first aid. She acknowledged the failure to provide proof that Staff #1 and #2 had first-aid training. A class was scheduled from an external service provider for 1/28/25, and all staff would attend. On 11/18/25 at approximately 5:00 p.m., the administrator on record acknowledged that proof of current first aid was not available.

A recertification survey with complaint #CO40403, #CO40659, #CO40794 and #CO40840 was completed on 1/21/26. A deficiency was cited. Based on records review and interviews, the facility (residence) failed to allow members (residents) to choose their own seat, choose their company (or lack thereof), and choose to converse (or not) for 19 current members (residents).Findings include:On 1/21/26 at 9:00 a.m., a diagram was observed posted near the dining area. On 1/21/26 at 9:01 a.m., a residence document posted read, in part, the member (resident) designated seating. On 1/21/26 at 4:30 p.m., Resident #1 stated that the residence had assigned seating. She acknowledged that if she wanted to sit by Resident #2, she couldn' t. On 1/21/26 at approximately 5:00 p.m., the acting administrator acknowledged they had seating arrangements for the residents. She added that the tables used to be in a straight line, with name tags to help with serving, but the residents liked it when they could sit in small groups to talk to one another, and they voted to draw popsicle sticks to find seating arrangements. She stated that they had never allowed residents to find their own seating. She acknowledged that it is the resident' s right to choose who they sit with.
