The seat was detached from the wall with exposed mounting screws, and the facility lacked an alternative shower chair for resident use. The care plan for Resident 6 required extensive assistance for all care except eating, and extensive assistance for transfers and toileting. However, the resident was found to have gotten up by herself without assistance and without using her call bell on 03/04/24.
During an observation on 05/10/24, the resident's call bell was located on the opposite wall behind her recliner, making it across the room from her bed. The call bell cord was not within reach of where the resident and wheelchair were positioned in front of the TV.
Multiple incidents occurred where the resident was found on the floor (01/26/24, 01/30/24, and 02/28/24) after attempting to use the bathroom or getting up. In one instance on 03/04/24, the resident presented with a bleeding area on her left arm following a fall.
