An initial licensure survey was completed on 10/1/25. A deficiency was cited. Based on observation, record review and interview, the residence failed to be in compliance with all applicable regulations.This failure created the potential for mismanagement of the care of and services for the residents who would be served by this residence.Findings include:-10.9 Each kit shall include, at a minimum, the following items: (A) Latex free disposable gloves, (B) Scissors, (C) Adhesive bandages, (D) Bandage tape, (E) Sterile gauze pads, (F) Flexible roller gauze, (G) Triangular bandages with safety pins, (H) A note pad with a pen or pencil, (I) A CPR barrier device or mask, and (J) Soap or waterless hand sanitizer.On 10/1/25 at approximately 10:28 a.m., the administrator provided the residence' s first aid kit. However, the kit was missing a cardiopulmonary resuscitation (CPR) barrier device or mask.On 10/1/25 at 10:28 a.m., the administrator stated she was unaware the first aid kit needed to include a CPR barrier device or mask.-13.1 The assisted living residence shall adopt, and place in a publically visible location, a statement regarding the rights and responsibilities of its residents. The assisted living residence and staff shall observe these rights in the care, treatment, and oversight of the residents. The statement of rights shall include, at a minimum, the following items: (A) The right to privacy and confidentiality. The residence' s undated Resident Rights policy, read in part: "The following rights are guaranteed by Colorado law (in accordance with) 6 CCR 1011-1, Chapter 7, §13.1 ... (residents have) the right to privacy."1. Statement of Rights During the environmental tour on 10/1/25 between 10:00 a.m. and 2:00 p.m., observation revealed the residence failed to adopt and place a statement of rights in a publicly visible location. On 10/1/25 at 11:03 a.m., the administrator stated she was aware of the requirement for resident rights to be in a publicly visible location; however, she wanted to consolidate her resident rights policy prior to posting, due to lack of space on her bulletin board. 2.
