Key Review Excerpts

Wonderful staff. Wonderful residents. Wonderful atmosphere. Very clean.

Visitor · 2017★★★★★

Weekends are SO loud in this facility. Even closing the door with your loved one you will still have to speak loudly to be heard 3/4 feet away. This seems like a "band-aid" place where patients arent necessarily taken seriously.

6deficiencies
Above state avg (4.3)

Heather Manor shows a pattern of recurring deficiencies across all three surveys, with medication management, resident rights, and infection control being the most frequent problem areas. The facility has consistently struggled with the same issues - medication labeling and management appears in every survey, and infection control deficiencies have persisted from 2023 through 2025. While all deficiencies show correction dates, the repeated nature of core problems suggests ongoing operational challenges that families should carefully consider.
