Specifically, for one of three sampled residents, there was no evidence of current licensed health professional assessment despite the resident having orders for fingerstick blood sugars and insulin administration.

The facility failed to ensure one of three sampled residents had completed the required two-step tuberculosis testing upon admission. Documentation for Resident #1 only showed an undated skin test from 2021 and lacked evidence of a completed two-step process.

Documentation only showed an old skin test date without clarification of what the date represented, and the administrator acknowledged the resident lacked a completed test at admission.

The facility failed to implement and document physician orders for blood pressure (BP) checks for two of three sampled residents. Specifically, for Resident #1, there were no BP readings or monitoring entries documented on the Medication Administration Record (MAR) for September, October, or November 2022 despite active orders for twice-daily checks.
