Based on documentation review, interview, and record review, the manager failed to ensure a caregiver provided documentation of completion of a caregiver training program approved by the Department or the Board of Examiners for Nursing Care Institution Administrators and Assisted Living Facility Managers (NCIA Board), for one of three caregivers sampled. Findings include: 1. A documentation review revealed a policy and procedure titled "Employees and Volunteers Qualifications" dated March 1, 2022. The policy and procedure stated, "A caregiver provides documentation of completion of a caregiver training program approved by the Department or by the NCIA Board." 2. In an interview, E1 and E2 reported E2 was a caregiver. E2 reported having documentation of completion of a caregiver training program approved by the Department or the NCIA Board. E2 reported E2 had been a caregiver for years at this location. 3. A documentation review revealed a series of personnel schedules dated between August 1, 2022, and August 9, 2023. The schedules revealed E2 worked alone as a caregiver multiple times each month between August 2022 and August 9, 2023. 4. A review of E2's personnel record revealed a caregiver certificate from White Dove Caregiver Training (ALTP0177) dated June 6, 2013. 5. A review of Department documentation revealed White Dove Caregiver Training was active from March 31, 2011, to March 31, 2013. The review revealed White Dove Caregiver Training was not an active caregiver training program on June 6, 2013, when the certificate was issued. 6. In an interview, a representative of the NCIA Board confirmed White Dove Caregiver Training was not an active caregiver training program when the certificate was issued. 7. In a separate interview, E1 and E2 reported not knowing White Dove Caregiver Training was not an active caregiver training program when the certificate was issued.
