A 30-day discharge notice issued to Resident A was deficient because it failed to specify the reason for discharge (from the four required categories) and failed to inform the resident of their right to file a complaint with the department.

Resident medications were not administered as prescribed; facility ran out of Tab-A-Vite iron tablets and failed to proactively reorder Amlodipine, resulting in missed doses. Additionally, staff documented medication refusals during periods when the medication was actually unavailable in the building.

Resident rights were violated as the resident was being kept in bed against her will/wishes. While a service plan mentioned rest, the resident reported being 'forced' to stay in bed, which decreased her strength, and staff indicated they would be in trouble if she stayed up.
