Improper storage in linen closets: The memory care soiled linen closet contained a Christmas tree; clean linen closets in the memory care and assisted living units contained therapy equipment, yard games, dusty fans, and cleaning supplies, posing a cross-contamination risk.
Food safety issue: Multiple food items in the kitchen refrigerator, freezer, and dry food storage areas (including memory care) were not dated or labeled with the open date.
Review of five employee records revealed all TB screenings were out of compliance (not completed within 10 days of hire or before occupational exposure, or records could not be found).
