The nurse did not lock my dads wheelchair and he ended up falling on the floor. The nurse that wrote a report lied in the report stating my dad fell onto his bottom which he actually fell on his hip and hit his head on the floor.

The staff allowed my family member to fall out of bed. They have no precautions for at risk residents for falls. In less than one week my family member had 2 falls, found crawling on the floor, left in recliner all night in soaked urine.

My mom was 82 years old when she went into the nursing home 10 months ago..lively, Sprite very funny lady with dementia... It is such a dirty place.. some good nurses but some pretty bad ones too..my mom was doped up with benzos to the point hospice thought she was actively dying.
