Key Review Excerpts

My husband was sent to this facility to help him get back on his feet after a stroke that was all wrong with him. My husband arrived on 1/27/26 and by 3/7/2026 my husband was almost died when I arrived.

Rehab patient's spouse · 2026☆☆☆☆

They kinked the hose in her oxygen tube and I fixed it only to come back the next day and find it kinked again. My wife was deprived of the proper amount of oxygen for at least one full day, most likely more.

Long-term resident's spouse · 2026☆☆☆☆

No notification when patient was transferred from Green Acres by Ambulance to Emergency room for Emergency care. No Follow up on welfare of said patient.
