Specifically, records for Resident #1 and Resident #3 lacked documentation of TB testing, and Resident #2 lacked documentation of a required second TB test.

Specific issues included stained linoleum, grime and mildew on bathroom walls and baseboards, rusted floor vents, and holes in the ceiling and walls. Additionally, several areas exhibited peeling paint, detached door hardware, and damaged wood surfaces.
