Specifically, records for Resident #1 and Resident #3 lacked documentation of TB testing, and Resident #2 lacked documentation of a required second TB test.

The facility failed to administer a medication as ordered for 1 of 3 sampled residents. Resident #2 did not receive Zyprexa 20mg as prescribed because the medication was missing from the pharmacy delivery, and the facility failed to notify the pharmacy of the omission.

Observations revealed multiple areas of peeling paint, exposed drywall, and the presence of black, gray, and black-spotted mold on window frames, wood trim, and walls. Additionally, spider webs were noted in the upper corners of the window.
