What this means for your family

This facility has a dedicated activity team and many staff members who are praised for their compassion. However, because multiple families have reported serious concerns regarding hygiene and medication management, we strongly recommend that you conduct an unannounced visit and ask specifically about their protocols for monitoring patient health and communicating with families during medical emergencies.

Google Reviews

Google Reviews

81 reviews analyzed
Grant Healthcare and Rehabilitation receives highly polarized feedback, with many recent reviews praising a compassionate, family-oriented staff and clean environment. However, these positive experiences are sharply contrasted by serious allegations from other families regarding neglect, medication management failures, and poor communication during medical emergencies. Prospective families should be aware of this inconsistency and conduct a thorough, unannounced visit to assess current standards.

Key Review Excerpts

Grant Healthcare And Rehab is a Place of love and real help! My mom came to them with late stage Lewy Body Dementia. To say that she is difficult does not begin to cover it. They never got to see this sweet, kind lady she was but they treated her as if they did.

Memory care family member · 2025★★★★★

The worst place to put your loved one. Only one good nurse out of the whole staff. My child was not taken care of. Left in urine, wasn't rechecked for a uti when she clearly had a bad one, was told they would check several times.

Long-term resident's family · 2024☆☆☆☆

Amy & Tiffany have been absolutely amazing. Bedside manager is top tier & they have been in constant communication. This is the best place to be at a time of need.

Near state avg (3.4)
1 complaint-triggered

Grant Healthcare and Rehabilitation has ongoing issues with fire safety and building systems, which have persisted across all three surveys from 2019 to 2025. The facility also had problems with infection control and resident protection from abuse, including one complaint-triggered investigation where families reported concerns. While all deficiencies show correction dates, the recurring pattern of fire safety violations and building maintenance issues suggests systemic challenges that families should discuss during their visit.
