The following deficiencies were found during the on-site investigation of complaint 00150363 conducted on November 13, 2025:
Based on the interview and documentation, the manager failed to ensure a resident was treated with dignity, respect, and consideration. Findings include: 1. In an interview, R1 reported to the compliance officer that E3 had previously yelled at R1 and had physically struck R1 on multiple occasions. R1 further reported that E3 expressed frustration about R1 ringing for assistance during the night, telling R1 that E3 “can’t get any sleep with the bell ringing.” R1 reported not being able to go into the patio area, and it is often discouraged by E3 and E4, even when the weather permits, and discouraged from leaving R1's bedroom. R1 reported not being treated with dignity, respect, and consideration. 2. On November 3, 2025, a report was submitted to our department, which alleged a caregiver was yelling and shouting at R1, and dragging R1 by the arm, and marks were visible on R1’s arm at the time of the incident. 3. In an interview, E2, E3, and E4 acknowledged each resident deserved to be treated with dignity, respect, and consideration.
