Questions for Your Tour

  • 1I noticed the facility is kept very clean and welcoming; what specific steps do you take to ensure that same level of care and professionalism is consistently reflected in the staff's daily interactions with residents?
  • 2Given that management and staff communication can sometimes be a challenge in nursing homes, how do you facilitate regular, transparent check-ins with families regarding their loved one's care?
  • 3With 16 recent state violations on record, could you walk me through the specific changes or training programs you have implemented recently to address these concerns and improve the quality of care?
  • 4What does a typical daily schedule look like for residents to ensure they stay socially engaged and active within the facility?
  • 5How is your medical team structured to handle urgent health changes or emergencies, especially during evening or weekend hours when staffing levels might shift?
  • 6I see that you occasionally engage with feedback online; how do you use that input from families to make tangible improvements to the resident experience here?

Key Review Excerpts

Loved meeting the staff and this is the best facility I have ever visited! Clean and healthy. Residents were awesome to be around!

Visitor · 2018★★★★★

The boss doesn't care for the health of their house keeping employees. The employee can have a appointment for the hospital and the boss would worry more about the rooms being clean then their employee. He plays favorite and it's always CNA and they aren't nice and always try to tell everyone what to do.

$132,380 in fines

This facility has a concerning pattern with 30 complaint-triggered deficiencies out of 47 total, indicating families have repeatedly filed reports about care issues. The most recurring problems involve resident safety and accident prevention, medication management, and protection from abuse and neglect. While all deficiencies show correction dates, the persistent nature of complaints about fundamental care areas like safety supervision and medication errors suggests ongoing quality concerns that families should carefully evaluate before placement.
