The facility failed to ensure that a registered nurse, physical therapist, or occupational therapist participated in quarterly on-site reviews and evaluations of residents' health status and care plans. This failure specifically related to tasks such as finger sticks and subcutaneous insulin injections.

The facility failed to implement and document physician orders for finger stick blood sugar checks as required. Specifically, for one resident, blood sugar checks were only being performed twice daily instead of the ordered frequency of before each meal and at bedtime.
