Observations showed sink temperatures at 96.6 degrees F and shower temperatures at 90.0 degrees F. The Administrator admitted to intentionally keeping temperatures in the 98 degree F range for safety.

The facility failed to ensure that a registered nurse, physical therapist, or occupational therapist participated in quarterly on-site reviews and evaluations of residents' health status and care plans. This failure specifically related to tasks such as finger sticks and subcutaneous insulin injections.
