The facility failed to maintain hot water temperatures at all resident fixtures between 100 and 116 degrees Fahrenheit. Observations showed sink temperatures at 96.6 degrees F and shower temperatures at 90.0 degrees F. The Administrator admitted to intentionally keeping temperatures in the 98 degree F range for safety.

The facility failed to ensure that a registered nurse, physical therapist, or occupational therapist participated in quarterly on-site reviews and evaluations of residents' health status and care plans. This failure specifically related to tasks such as finger sticks and subcutaneous insulin injections.

Additionally, the Medication Administration Records (MARs) were not updated to reflect the correct frequency of these required checks.
