FACILITY EVALUATION REPORT

The facility underwent an unannounced Case Management visit to follow up on a Special Incident Report (SIR) concerning an incident that occurred on 02/04/2026. The Licensing Program Analyst reviewed facility records and interviewed the Designated Administrator. No deficiencies were cited during this case management visit.

The inspection identified four deficiencies, including two Type A citations representing immediate risks. Key issues found were the unsecured exit gate, medication documentation errors on the EMAR, improper storage of medications, and failure to maintain updated resident reappraisals. Immediate corrective action is required for the Type A violations.

The facility underwent an unannounced annual visit on 09/26/2024. The LPA reviewed resident and staff files, observed the facility's various wings, and checked safety equipment. Although a technical violation for Section 87312 was provided, no formal citations were issued during this visit.

The facility was visited for a Case Management review following an incident report of an unwitnessed fall. The evaluator reviewed records and observed lunch service, noting that no deficiencies were cited as a result of this specific visit. The report confirms prior citations regarding signal system inoperability, for which an extension was granted.

The facility was inspected on 10/17/2023 as a Plan of Correction (POC) visit to review deficiencies from a prior annual visit. The report explicitly states that all deficiencies cited under Title 22 Regulations have been cleared, and the licensee complied with the terms of the POC by the due date. No deficiencies were observed or cited during this visit.
