A relicensure survey was completed on 4/9/25. A deficiency was cited. Based on record review and interview, the residence failed to comply with authorized practitioner orders associated with medication administration, except for those medications which a resident self-administers, affecting one of three current residents (#3). Findings Include:Resident #3 was admitted to the residence on 2/3/25.A written practitioner' s order, dated 3/11/25, directed the residence to administer levothyroxin 50 mg once daily. However, the blister pack of levothyroxin was labeled as 75 mg instead of 50 mg. Therefore, the resident was administered the incorrect doses from 3/27/25 through 3/31/25.On 4/8/25 at approximately 1:52 p.m., a medication cart audit was conducted, and Resident #3 was found to have levothyroxin 75 mg. However, the March 2025 medication administration records (MAR) for Resident #3 read 50 mg once daily.InterviewsOn 4/8/25 at approximately 2:37 p.m., the administrator stated she expected the residence to administer medications according to the practitioner' s orders. Additionally, she stated that the "girls pulled from the overflow" medications and incorrectly administered 75 mg instead of 50 mg levothyroxin without a practitioner' s order.On 4/9/25 at approximately 8:14 a.m., the lead qualified medication administration person (QMAP) stated she expected QMAPs to review the MAR and administer the correct .. THIS PORTION OF THE REPORT IS FOR INFORMATIONAL PURPOSES ONLY.No response is necessary.The residence was advised that it must review and maintain the following processes in accordance with existing program regulations found at 6 CCR 1011-1, Chapter 7.10.1 A readily available roster of current residents, their room assignments and emergency contact information, along with a facility diagram showing room locations.16.24 Potentially hazardous foods that are stored hot shall be held at or above 135°F. Assisted living residences can achieve this by keeping soups, sauce, and other hot foods warm on a stove burner, in the oven, or on a warming plate at a temperature above 135°F until they are served, stored, or discarded.22.3 Cleaning compounds and other hazardous substances (including products labeled "Keep out of reach of children" on their original containers) shall be clearly labeled to indicate contents and (except when a staff member is present) shall be stored in a location sufficiently secure to deny access to confused residents. 22.24 Toilet paper in a dispenser, liquid soap, and paper towels or hand drying devices shall be available at all times in each common bathroom.
