Questions for Your Tour

  • 1With 18 recent state violations on record, what specific steps has the leadership team taken over the last year to improve facility cleanliness and sanitation protocols?
  • 2I noticed the staffing rating is currently lower than the overall CMS rating; how do you ensure consistent, attentive care for residents during shift changes and weekends?
  • 3Communication is very important to our family; what is your standard process for keeping families updated on changes in a resident's health or daily status?
  • 4We have read that personal belongings can sometimes go missing in facilities; what security measures or labeling systems do you have in place to protect a resident's personal property?
  • 5Given the mixed feedback regarding nursing responsiveness, how do you empower your staff to ensure that call bells are answered promptly and resident needs are met in a timely manner?
  • 6I see you have some dedicated staff members and active recreational programs; could you walk us through a typical week of activities and how you encourage residents to participate?

This facility has concerning patterns of complaint-driven problems, with families filing reports that led to nearly half of all deficiencies found. The facility shows recurring issues with abuse and neglect investigations, basic safety and care oversight, and resident rights violations. While inspectors indicate corrections have been made, the persistent family complaints across multiple years about fundamental care quality and safety supervision suggest ongoing challenges families should carefully consider before choosing this facility.
