Number of residents present at the facility at the beginning of the inspection: 8 The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. Number of resident records reviewed: 2 Number of staff records reviewed: 3 Number of interviews conducted with residents: 8 Number of interviews conducted with staff: 2 Observations by licensing inspector: Physician?s orders, medication administration, medication administration records, medications. First aid kit supplies, emergency food and water. All residents were neat, clean and well groomed. All 8 residents were interviewed regarding care and supervision at the facility and no one voiced any concerns or complaints to Licensing staff during the inspection. Facility was also clean. Lunch meal was being prepared. All required postings were observed. Liability insurance declaration is current. Additional Comments/Discussion: Inspector was accompanied by (Supervisor)- Licensing Administrator for Central Region. All required postings observed. Staff members were given the opportunity to ask questions. Staff informed Inspector that the residents case manager is in the facility monthly to monitor the resident's care and recommend any interventions that may be needed. An exit meeting will be conducted to review the inspection findings. The evidence gathered during the inspection determined no violations with applicable standard(s) or law. The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law. VIOLATION: Based upon the building tour, it was noted that all medication failed to be store in a locked area. EVIDENCE: During the building tour on July 29, 2022, it was noted that room #6 had a tube of Betamethasone Valerate cream1% that was left lying on the resident?s nightstand.
VIOLATION: Based upon the medication administration observation, it was noted that the facility failed to administer all medications in accordance with the physician?s or other prescriber?s instructions. EVIDENCE: During the August 06, 2022, medication pass observation it was noted that resident #3 was not administered Ibuprofen 600 MG as per the prescriber?s instruction. The facility failed to provide a D/C order or a new physician order that would identify a change in how the resident?s medication would be administered or why the resident failed to receive the medication as prescribed.

An unannounced monitoring inspection was conducted on April 19, 2022, from 3:20 p.m. to 4:34 p.m. for First Choice South Brunswick Assisted Living Facility. At the time of entrance, the facility?s infection control monitoring policy was actively implemented for all visitors, staff members, and residents. The facility administration team reported the census as seven (7) residents in care. The record sample size consisted of two (2) resident records and two (2) new staff records, as well as other documents reviewed such as menus, staff schedules, staff certifications, training records, activities schedules, health inspections, and fire inspections. The licensing inspector and the designated staff member toured the building to observe the facility?s commons areas, bathrooms, and bedrooms. VIOLATION: Based upon the record review, the facility failed to review the rights and responsibilities with each staff member. EVIDENCE: Staff record #2 failed to have evidence that the rights and responsibilities were reviewed annually. The most recent rights and responsibilities review dated in record #2 was 9/10/2020.

An announced initial inspection was conducted at the facility on November 22, 2021 by licensing staff from 10:45 a.m. to 1:00 p.m. A tour of the facility was conducted which included the following observations: building and grounds, facility postings and the first aid kit.
