As soon as you walk in to Fairmont you are greeted by the front desk as if you are on a 5 star luxury resort. The place is clean, although Fairmont is 5 years old it still has the new place feeling, smells great, the residents are well taken care of, and staff is extremely friendly.

The staff were wonderful in their care. At this point she was not eating and she passed shortly afterwards. I cannot thank the staff that cared for her and Americare Hospice enough for the care and support that was given during her short stay.

The following deficiency was found during the on-site compliance inspection and investigation of complaint AZ00198439, AZ00198712, AZ00200974, AZ00202102, AZ00203270, AZ00205511, and AZ00208470 conducted on May 08, 2024:

If an assisted living facility provides medication administration, a manager shall ensure that:R9-10-816.B.3.bCorrected May 21, 2024

Based on record review and interview, the manager failed to ensure a medication was administered to a resident in compliance with a medication order, for one of thirteen residents sampled. The deficient practice posed a risk if the resident experienced a change in condition due to improper administration of medication. Findings include: 1. A review of R3's medical record revealed a service plan for directed care services that included medication administration. 2. A review of R3's medical record included a medication order for "HYDRALAZINE 25 MG TABLET TAKE 1 TABLET BY MOUTH ONCE AS NEEDED. HOLD FOR (Systolic blood pressure ) SBP LESS THAN 110 HR GREATER THAN 100." 3. A review of R3's medical record revealed on January 19, 2024 at 5:18 PM the medication HYDRALAZINE 25 MG TABLET was administered. However, at the time the SBP was 98. 4. In an interview, E1 acknowledged R3's medication was not administered in compliance with the available medication order.
