Key Review Excerpts

The housekeeping is terrible. It smells. The only good thing I can say is the nursing and CNA staff are caring.

4deficiencies
Above state avg (3.9)

This facility has 28 deficiencies across three surveys with recurring issues in medication management, fire safety systems, and food safety protocols. The most concerning pattern is repeated food safety violations in all three surveys and ongoing building/electrical problems. While all deficiencies show correction dates, the recurring nature of key safety issues like medication errors and food handling suggests potential systemic challenges that families should discuss during visits.
