What this means for your family

While the facility offers strong rehabilitation services and a newly renovated environment, the reports of neglectful night-shift care and sanitation issues are deeply concerning. We strongly advise visiting during off-hours to observe staffing levels and cleanliness for yourself, and to specifically ask how the facility handles hygiene requests for immobile residents.

Evergreen Post Acute has undergone recent management changes and physical renovations that have garnered praise from some residents and family members for improved aesthetics and staff engagement. However, the facility faces significant, recurring reports of poor hygiene, neglectful night-shift care, and inconsistent responsiveness to basic patient needs. Families should be aware of a stark divide between positive experiences with rehabilitation services and serious allegations regarding cleanliness and compassionate care.

Key Review Excerpts

The nursing and therapy teams are outstanding. Whether it's physical, occupational, or speech therapy, the personalized attention and dedication to recovery are evident.

Rehab patient · 2025★★★★★

Many of the staff at this place are horrible. My brother is terminal, he is on hospice. He had a bed wetting accident during the night. He asked the staff if they could change his bedding. They refused. He had to sleep on the floor from midnight until the day shift came in.

Family member of terminal patient · 2025☆☆☆☆

The staff comradery really helps the facility feel amazing. I loved my time here at evergreen post acute. I git great rehab here. The therapist all care a lot and take the time to help me improve.

Families have filed complaints that triggered inspections, raising concerns about protection from abuse and neglect. Evergreen Post Acute shows recurring deficiencies in resident care, medication management, and food service across multiple surveys from 2022 to 2025. While all violations are marked as corrected, the pattern of repeated issues in core care areas, combined with complaint-driven investigations, suggests ongoing quality challenges that warrant careful consideration during visits.
