Evergreen Assisted Living II LLC

Based on documentation review, observation, and interview, the governing authority failed to designate a manager in writing. The deficient practice posed a risk as the assisted living facility did not have a manager for approximately 20 days. Findings include: 1. A review of Department documentation revealed an email from E2, dated February 27, 2025, indicating E2 was no longer the manager of the facility, effective the same day. The review revealed no email or other notification regarding the identity of the manager after E2. 2. The Compliance Officer observed no posted manager's certificate in the facility. 3. In an interview, E1 reported the facility had not had a manager since E2 removed E2’s certificate on February 27, 2025. E1 stated E1 was “in the process” of finding a new manager. This is an uncorrected deficiency from the complaint inspection conducted on December 19, 2024.
