Based on record review and interview, the assisted living home failed to maintain a standardized form for each resident that included the information prescribed in A.R.S. § 36-420.04.A.1-9 for two out of two residents sampled. The deficient practice posed a risk if the facility was not prepared in case of an emergency. Findings include: 1. A review of R1's medical record revealed a standardized form to be used if an emergency responder was contacted, however, the form was missing the following information: The point-of-contact information for the assisted living center or assisted living home, as well as the telephone number, if available, cell phone number and email address; and Basic information about the resident's physical and mental conditions and basic medical history, such as having diabetes or a pacemaker or experiencing frequent falls or cardiovascular and cerebrovascular events, as well as dates of recent episodes, if known. 2. Based on documentation review and interview, the manager failed to ensure that a disaster drill for employees was conducted on each shift at least once every three months and documented. The deficient practice posed a risk if employees were unable to implement a disaster plan. Findings include: 1. A review of the facility's documentation revealed a disaster drill conducted on November 12, 2024, on February 12, 2025, and on May 12, 2025. However, the drills were only being conducted on one shift (1st shift). 2. A review of the facility's documentation revealed the facility had two scheduled shifts, 7am-7pm (1st shift) and 7pm-7am (2nd shift). 3. In an exit interview, the findings were reviewed with E3 and no additional information was provided.
