The facility has committed to having physician-reviewed diets matched to facility diets upon admission and conducting semi-annual reviews of diets by the Administrator.

The facility failed to ensure that 2 of 3 sampled residents had completed the required two-step tuberculosis (TB) testing upon admission. Specifically, documentation for Resident #2 and Resident #3 only showed the first step of testing, with no evidence of the second step being completed.

The facility failed to administer medications as ordered by a licensed prescribing practitioner for Resident #2. Specifically, the resident had an order for ketoconazole 2% cream once daily to treat a fungal infection that was not being properly administered.

Additionally, dust accumulation was noted on air vents and dirt was present in various wall corners and room areas.
