The facility failed to ensure that resident assessments and LHPS were consistently completed. The Administrator has implemented a plan to conduct quarterly reviews of resident records in conjunction with pharmacy reviews to ensure compliance.
There was a lack of consistent monitoring to ensure resident diets matched facility capabilities and physician orders were followed. The facility has committed to having physician-reviewed diets matched to facility diets upon admission and conducting semi-annual reviews of diets by the Administrator.

The facility failed to administer medications as ordered by a licensed prescribing practitioner for Resident #2. Specifically, the resident had an order for ketoconazole 2% cream once daily to treat a fungal infection that was not being properly administered.
