This report details a complaint investigation conducted on December 3, 2024, regarding an allegation of an uncleared adult working at the facility. Based on observations, interviews, and record review, the department found the complaint to be UNFOUNDED. No deficiencies were issued during this investigation.

The Licensing Program Analyst met with the Assistant Administrator and noted that routine screening procedures, including temperature checks and the use of PPE, are in place and being followed.
