Key Review Excerpts

I was a patient for 7 weeks recovering and rehabbing from a below the knee amputation. As this was my first experience with a Skilled Nursing facility, I was apprehensive at first. I arrived with an open mind and was amazed with the level of care I received. Almost every staff member makes you feel how much they care about the residents. My physical and occupational therapists (Wednesday and Steven) were amazing.

Rehab patient · 2025★★★★★

My mom was placed at this facility for 5 weeks, towards the end of her life , and she got nothing but exemplary support and care from everyone. I was around almost every other day and the facility was always super clean and I saw a lot of activities going on for the residents that lived there.

Long-term resident's family · 2025★★★★★

We're impressed by how much the team cares about their patients and how well they are treated and how great their environment is. My dad was in the process of slowly recovering from a neurological condition that was causing him unable to function properly on his own and was needing a lot of attention to his physical condition and the way they were able to manage his health was amazing.

This facility has concerning patterns with families filing complaints about abuse reporting and safety issues that persist across multiple surveys. The most recurring problems involve resident assessment and care planning, food safety and nutrition services, and fire safety systems. While all 44 deficiencies across 6 surveys show correction dates, the repeated issues with resident safety, dietary services, and failure to properly report suspected abuse suggest ongoing operational challenges that warrant careful consideration during your visit.
