Questions for Your Tour

  • 1With your staffing rating currently at one star, what specific steps are you taking to ensure that residents receive timely assistance with daily personal care and hygiene needs?
  • 2I noticed some concerns regarding the consistency of physical therapy and social programming; could you walk me through how you track and guarantee that these scheduled activities actually take place for each resident?
  • 3Given the facility's history of two state violations in the past year, what changes have been implemented to improve your internal oversight and quality control processes?
  • 4Since you offer a convenient transition between levels of care, how do you ensure that a resident’s care plan remains accurate and responsive as their medical needs evolve?
  • 5How does your team handle urgent medical situations or sudden changes in a resident's health status during the night or on weekends when staffing levels are typically at their lowest?
  • 6What is your current protocol for maintaining the cleanliness of resident rooms and common areas throughout the day to ensure a comfortable living environment?

Key Review Excerpts

My wife put her mom there Building E and wasn't a good idea they didn't change her briefs, her mom got infection from unprofessional cleaning job from the caregivers and her mom end up in the hospital for three weeks because of the infection, UTI as well.

Memory care family member · 2025☆☆☆☆

We were we very sorry we put our mom in this facility. We were told they would do activity’s with her which never happened. Her clothes were constantly dirty.

Memory care family member · 2022☆☆☆☆

My parents have lived here since it opened 1999. Great community, staff is awsome. And now with the new nursing home close, makes it very easy to transist from an illness to assisted living.
