Questions for Your Tour

  • 1Given the recent feedback regarding response times, could you walk me through your current process for ensuring residents receive timely assistance when they press their call lights?
  • 2I noticed the facility has a strong rehab therapy team; how do you coordinate that care with the nursing staff to ensure consistency in a resident's daily routine?
  • 3Could you explain your current protocols for medication administration and how you verify that doses are being delivered accurately and on time?
  • 4With your proximity to the local hospital, what is your specific process for managing urgent medical changes or emergencies that require a higher level of care?
  • 5Regarding personal hygiene and grooming, what is the standard schedule for resident showers, and how do you monitor that these essential daily needs are consistently met?
  • 6I see that you have a 4-star staffing rating, but some families have expressed concerns about safety and fall prevention; what specific measures or equipment, such as bed alarms, are standard for residents at risk of falling?

Key Review Excerpts

The facility didnt even know nothing about my mom such as her having a bed sore coming up which has now turned into full bed sore with infection. They had her over medicated and she couldn't even speak to tell anyone she was sitting in her own feces.

Long-term resident's family · 2026☆☆☆☆

It is understaffed and does not have adequate safety measures (such as bed alarms) in place. My mother's roommate, who was there because of a broken hip, crawled out of her bed falling onto the floor ten times while I was there.

Long-term resident's family · 2024☆☆☆☆

Having the hospital next door was also a blessing. Such a comfort to know if she needed medical assistance it was mere footsteps away.

Families recently filed complaints about resident abuse and neglect that led to federal citations in July 2025, raising serious safety concerns. The facility shows recurring problems with fire safety systems, food safety, and resident protection that have persisted across multiple surveys from 2018 to 2025.
