Delmar Gardens South receives highly polarized feedback, with some families praising the dedicated nursing staff and engaging activities, while others report severe neglect and communication failures. Recurring complaints highlight significant issues with call light response times, hygiene, and medication management, particularly during off-hours and weekends. Families should be aware that while some residents thrive in the rehab and long-term care programs, others have experienced distressing lapses in basic care and administrative accountability.

Questions for Your Tour

  • 1I noticed your facility offers a wide variety of daily activities; could you walk me through how you help new residents get integrated into these programs and ensure they stay socially engaged?
  • 2Given that medication management is a critical part of care, could you explain the specific protocols and double-check systems you have in place to ensure accuracy for residents?
  • 3I see that you are active in responding to feedback online; how does your administrative team use that family input to improve daily operations and communication?
  • 4When a resident uses their call light, what is the standard procedure for ensuring a timely response, and how do you monitor those response times to ensure needs like hydration and hygiene are consistently met?
  • 5Since your therapy team is highly regarded, how do they collaborate with the nursing staff to ensure a resident's physical progress is maintained during their daily routine?
  • 6How does your leadership team proactively communicate with families regarding changes in a resident's health status or care plan to ensure we are always kept in the loop?

Key Review Excerpts

The physical therapy and occupational therapy folks are tremendous. I feel very comfortable that all my wife's needs are being tended to.

Long-term resident's family · 2024★★★★★

My father was admitted to this facility for rehabilitation... On multiple visits, we found him in a soaked adult diaper. He couldn’t feed himself due to severe motor impairments, yet we frequently found food placed out of his reach, and he rarely had access to water.

Rehab patient's family · 2025☆☆☆☆

My grandmother’s medications were mishandled multiple times. At one point on Wednesday, Feb. 11th, the nurse apologized and admitted she had forgotten her glasses and did not see the fine-print instructions on the medication.
