The facility underwent an unannounced Case Management visit. One Type A deficiency was cited regarding the failure to secure a criminal record transfer clearance for an individual. Additionally, one Type B deficiency was noted concerning the incomplete association and criminal transfer documentation for a staff member.

This report details a complaint investigation conducted on February 28, 2026, regarding allegations of rough handling of a resident. The investigation found that there was not a preponderance of evidence to prove the alleged violation occurred, resulting in the allegation being unsubstantiated.

The facility underwent a required annual inspection and was generally found to be compliant in many areas, including infection control and physical plant safety. However, three deficiencies were noted: a required repair to an eave near stairway #3, an update needed for the emergency disaster plan to include relocation sites, and missing labels on two resident PRN medications. All deficiencies cited were Type B, indicating non-compliance that could become a risk if not corrected.
