The following deficiency was found during the on-site compliance inspection and investigation of complaint 00132186 conducted on February 5, 2026:

Based on observation, record review, documentation review, and interview, the manager failed to ensure that a caregiver's and assistant caregiver's skills and knowledge were verified and documented before providing physical health services, according to policies and procedures, for one of two employees sampled. The deficient practice posed a risk if the employees were unable to meet a resident's needs. Findings include: 1. During the on-site inspection, the Compliance Officers observed E2 assisting residents to the dining room table for lunch. 2. A review of E2’s personnel record revealed no documentation that E2's skills and knowledge were verified and documented before providing physical health services. Based on E2’s hire date, this documentation was required. 3. A review of the facility’s policies and procedures revealed a policy titled "Caregiver and Volunteer.” The policy and procedure stated, "The skills and knowledge are verified by manager at the time of orientation, before providing assisted living services to a resident." 4. In an exit interview, the findings were reviewed with E1 and E5, and no additional information was provided.

Based on this deficiency-free compliance inspection, the Department shall not conduct a compliance inspection for twenty-four months, according to A.R.S. \'a7 36-425(E). Subsection (E) does not prohibit the Department from enforcing licensing requirements as authorized by A.R.S. \'a7 36-424.
