The facility failed to notify the Division of Health Service Regulation (DHSR) regarding changes in residents' evacuation capabilities. Specifically, two out of six sampled residents were found to have evacuation capabilities that differed from the ambulatory status listed on the facility's license, as evidenced by their inability to exit the facility during a fire drill.

The facility failed to provide clean top and bottom sheets for a resident's bed. Specifically, a resident using a bariatric bed was found laying directly on the mattress without sheets because the facility had not secured appropriately sized linens.

The facility failed to maintain a current weekly menu in the kitchen for the guidance of food service staff. Observations revealed no weekly menu was available for staff to reference while preparing meals.

The facility failed to maintain the front porch in a clean and safe condition. Observations revealed loose, weathered, and peeling stair rails, detached wooden boards, and loose or missing porch spindles.

The facility failed to ensure the kitchen was clean and protected from contamination. Findings included brownish spots under the stove hood, food debris and flying insects in the dishwasher, and dark residue inside the oven.

The facility failed to maintain walls, ceilings, floors, and floor coverings in a clean and good state of repair. Specific issues included stained carpeting in the living room, cracked walls and broken baseboards in the hallway, and multiple holes and peeling paint in resident bathrooms.
