The facility failed to ensure that three exit doors had a continuously sounding device activated when opened, which is required for residents who are disoriented or exhibit wandering behavior. Observations revealed that the alarms for the facility entrance and two other exit doors did not sound continuously. This deficiency affected several residents, including those identified as intermittently or constantly disoriented and one resident who wandered.
