What this means for your family

The facility has recently undergone a management change and shows strong potential in therapy and resident engagement. However, given past reports of communication lapses regarding patient safety and slow call-light response, I strongly recommend asking for a tour and specifically inquiring about their current protocols for incident reporting and staffing ratios during night shifts.

Key Review Excerpts

Daddy called me bragging on his nurse this morning. That's a real good sign because he wasn't comfortable enough to say much good at the hospital. So I know he is a lot more comfortable here!

Long-term resident's family · 2025★★★★★

My mother had two falls one night and we were not notified. We actually found out from my mother's roommate. The facility is very ran down and dated. Call lights often go unanswered or go over for periods greater than 30 minutes.

Long-term resident's family · 2024☆☆☆☆

She was always taken good care of by the staff, until her passing. I would recommend this facility for anyone needing care for their loved one.

Families have filed reports triggering seven deficiencies, including a recent serious allegation of abuse and ongoing medication errors. The facility shows persistent problems with medication management, fire safety systems, and care planning across multiple surveys. While all deficiencies show correction dates, the recurring pattern of issues in core care areas and recent complaint-triggered violations for abuse and medication errors warrant careful consideration before placement.
